Initial appointment: This visit is 45 to 60 minutes.  Please email or bring any recent lab results with you as well as a three-day food record.  At this time your nutritional status will be evaluated.  Your medical and diet history  will be reviewed.  Based on this information an individualized strategic nutrition plan will be created to help you reach your overall health goals.

Email back to lisa@lisastollmanrd.com before your first visit. 


Follow-up visits: These visits are 30 to 60 minutes.  At each of these visits we will evaluate where you are in meeting your goals.  Your food diary, weight and any recent labwork will be evaluated.  Strategies will be discussed to help you in achieving your goals.


Cancellation Policy:
If you need to cancel please due so at least 48 hours before the scheduled appointment.  Otherwise you will be charged $200.00 for the visit.  Our time is also valuable.  Please be considerate.

Medicare (diabetes and kidney disease only--physician referral required)


Please note:  you must contact your insurance company to check that you have coverage for nutrition services for your specific diagnosis before your first appointment.  If you do not check with your insurance provider prior to the first appointment, you will be responsible for payment at the time of the visit.  If you need a referral, please bring it along with you to the first visit.  You will be responsible for any visits which your insurance carrier denies.


Self-pay
Self-pay for nutrition services are as follows:
Initial visit (60 minutes) $350.00   
Follow-up visit  (30 minutes) $150.00/(60 minutes) $300.00

Payment is due at the time services are rendered. We accept cash, checks and credit cards.
