Self-Pay
Self-pay for nutrition services are as follows: Initial visit (60 minutes) $300.00 Follow-up visit (30 minutes) $150.00/(60 minutes) $300.00
Payment is due at the time services are rendered. We accept credit cards, Venmo, and Zelle.If customers can’t find it, it doesn’t exist.
Insurance Plans We Accept
We accept BCBS, NYSHIP, Medicare (for diabetes and kidney disease only), New York State Medicaid.
WE accept Aetna and Cigna via the Berry Street platform. Here is the link to make a visit if you have either of these insurance plans: visit.berrystreet.co/providers/Lisa-Stollman.
Please note: you must contact your insurance company to check that you have coverage for nutrition services, for your specific diagnosis, before your first appointment.
If you do not check with your insurance provider prior to the first appointment, you will be responsible for payment at the time of the visit. If you need a referral, please bring it along with you to the first visit. You will be responsible for any visits which your insurance carrier denies.
